Covenant Community Care, Inc. is a faith based, Community Health Center dedicated to providing primary health care to the uninsured and under insured of Southeast Michigan. Our mission is to show and share the love of God, as seen in the good news of Jesus Christ, by providing integrated, affordable and quality health care to those who need it most.

PHYSICIAN/PHYSICIAN IN CHARGE

The physician in charge is responsible for the coordination of clinic activities such that the delivery of medical services promotes efficient and effective medical care according to the mission, vision and goals of Covenant Community Care, Inc. and will serve as liaison between Beaumont Hospital and Covenant Community Care to assure quality and a smooth transition for patients between these two care settings.

 

FAMILY NURSE PRACTITIONERS

The Certified Nurse Practitioner (CNP) is responsible for supporting the physician in the delivery of medical services to promote efficient and effective medical care according to the mission, vision and goals of Covenant Community Care, Inc. The CNP will function as an integral member of a collaborative practice and will participate in the care of newborns through the elderly with physicians.

 

AND A CERTIFIED NURSE MIDWIFE 

The Certified Nurse Midwife (CNM) will serve as a physician extender to Ob-Gyn staff service patients, exclusively for outpatient services. The CNM will provide comprehensive, primary health care to a select population of essentially healthy women. The CNM  will provide direct patient care, including history and physical exams, ordering and evaluation of tests, individualized patient teaching and counseling, and management of medical conditions of Ob-Gyn Clinic patients. The CNM will function as an integral member of a collaborative practice and will participate in care of patients with physicians. Care will be consistent with practice standards, i.e., ACOG and American College of Nurse Midwifery, and according to the mission, vision and goals of Covenant Community Care, Inc.
